Lambdify misinterprets some matrix expressions

Verdict: the fix is stated in the issue (Tier A).

If the shape is an explicit number, we can just print `eye(n)`. For unknown shape, it's harder. We can raise an exception for now. It's better to raise an exception than give a wrong answer.
